Welcome to Believe Church! If you're visiting us for the first time, please complete this family registration update form so that your child/children can participate safely in our programs. Once this is complete, your child can be checked into the Believe Kids program. This form applies to all children under 18 and will cover our Believe Kids and Believe Youth programs.

Your information is covered by our Privacy Policy and we promise not to add you to a database unless you choose to become a regular member of our church programs in the future.

I authorise the Believe Kids and/or Youth leaders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child.

I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behavior may result in my child being sent home and being temporarily or permanently prohibited from attending the activities of Believe Kids and/or Youth.
